Students are required to complete the Eligibility Form to be reviewed for eligibility to apply to the Nursing Program - Generic and Advanced Placement.  You must have a current VCCCD Student ID number to complete the form.  The Eligibility Form must be complete and accurate to determine eligibility.

Have you completed all required prerequisites?
There are 6 required prerequisites – math, chemistry, anatomy, physiology, microbiology and current California CNA or Psych Tech or LVN;  30 unit option only requires physiology, microbiology and current California LVN license.
Do you have current California certification as a Nursing Assistant (CNA) or current CA Psych Tech or CA LVN license?
Have you attended any other regionally accredited colleges/universities other than Ventura, Oxnard, or Moorpark College?
Must list ALL colleges/universities attended and official transcripts must be on file. Please list all colleges individually even if part of a district – for example DO NOT just list LA Community College District; student must list all colleges attended within the LACCD.
Have you ever taken the TEAS exam ATI TEAS 6 or ATI TEAS 7? If yes, list date(s) and Total Score(s).
